Wellthy End of Life Planning & Support

Whether you’re being proactive and planning ahead, caring for a loved one through their end-of-life journey, or facing a sudden loss, Wilbur-Ellis understands your need for compassionate support. When you enroll in Sun Life’s life insurance, you’ll find caring experts through Wellthy. The program is available to you, your beneficiary, and your family members.

Wellthy’s services include:

• Planning for the future: The program features resources and tools for will preparation, advanced directive, healthcare power of attorney, checklists, and guidance for having end-of-life planning conversations with family and friends.
• Navigating end of life care: Comfort care providers (hospice, palliative care, doulas, etc.) will be vetted for you; medical appointments will be scheduled and tracked, as well as arrangements being made for respite care for the caregiver; and insurance coverage for comfort care will be verified.
• Managing a loss: The program includes reviewing funeral and burial options and coverage (including through the Veterans Administration), supporting families through the first steps immediately after a loss, helping with legal and financial support resources, and providing guidance for identifying/resolving outstanding bills and insurance claims.
• Providing support following a loss: Wellthy’s care coordinators will locate bereavement support (counseling, therapy, support groups), share resources and advice for coping, self-care, and emotional well-being, and arrange meal and grocery delivery for grieving families.
• Supporting pregnancy and postpartum loss: Lactation after loss services and resources will be provided by your care coordinator, who can also schedule appointments with the right providers and specialists for both parents.

To get started, follow these steps:

• Create an account by visiting com/member/sunlife-yourjourney and click “ACTIVATE MY ACCOUNT.” Enter your name, email, and a password. You’ll need access to your email to verify your account.
• Confirm your eligibility by checking the box to verify you are either a Sun Life life insurance member, a beneficiary, or a loved one caring for the life insurance member.
• Get started by choosing whether you’d like to begin working with a dedicated Care Coordinator, explore Wellthy’s Care Planning tool to prepare for the future, or join Wellthy Community—a space where you can connect with others who may be going through something similar.
